New Employee Technology Requisition
Use this form to request technology access for a new permanent or temporary staff member. The form should be filled out electronically, then printed and forwarded to HR with the rest of the new employee's hiring paperwork. This form includes the Data Access Request Form.
Data Access Request Form
Use this form to request access to Datatel, Workday or Salesforce for an existing employee. The form should be filled out electronically, then printed, signed, and interofficed to the Helpdesk / ETS-Oakland or emailed to firstname.lastname@example.org.
Employee Termination Technology Checklist
Use this form to remove email, network, and other access for terminated employees. This form should be filled out electronically by the Hiring Manager, printed and signed, and then forwarded to HR with the rest of the terminated employee's paperwork. For immediate access revocation, please contact the Helpdesk at 510.594.5010.
Data Security and Confidentiality Agreement
Use this form for all new and existing employees to ensure knowledge about and agreement to ETS policies surrounding Data Security and Confidentiality. This form should be signed physically and returned to the Helpdesk.
Administrative User Responsibilities Agreement
Use this form to request administrative responsibilities for your CCA-provided computer. This form must be filled out with User Support Staff and signed by both parties. Please contact the Helpdesk at 510.594.5010 for more information.
